Medical Authorization Forms
If your child needs to take medication during the school day, California state law requires a Medication Authorization Form to be on file each school year.
This applies to:
Prescription medications like EpiPens, inhalers, or antibiotics
Over-the-counter (OTC) items such as Tylenol, Advil, allergy medicine, cough drops, eye drops, lip balm, sunscreen, and more
All medications must be stored in the school nurse’s office, except for:
Asthma inhalers and sunscreen, which may stay in your child’s backpack (but still require a doctor’s note for our records)
Required Forms:
Medication Authorization Form — required each school year
FARE Form — required if your child uses an EpiPen
